Notice is hereby given that pursuant to Chapter 373, Florida Statutes, the following application for water use permit has been received by the Northwest Florida Water Management District:

Interested persons may submit written comments/objection or submit a written request for the notice of proposed agency action (NOPAA) regarding the application by writing to: Division of Resource Regulation, Northwest Florida Water Management District, attn.: Terri Peterson, 152 Water Management Drive, Havana, Florida 32333. A NOPAA will be mailed only to persons who have filed such requests. A NOPAA must be requested in order to be advised of further proceedings and any public hearing date. Written comments/objections or NOPAA request must be received by 5:00 p.m. eastern time on October 26, 2012

No further public notice will be provided regarding this application. Publication of this notice constitutes constructive notice of the permit application to all substantially affected persons.

REQUEST FOR LETTERS
OF INTEREST

The Calhoun County Board of County Commissioners will receive Letters of Interest from any qualified engineering and surveying firm interested in providing the requested services for the following project:

ALTHA SIDEWALK
CONSTRUCTION ENGINEERING
AND INSPECTION SERVICES

The project will consist of construction engineering and inspection of the sidewalk and minor drainage improvements along Smith Street, Fuqua Circle and North Street within the Town limits of Altha in accordance with FDOT Standards as identified in the County’s Local Agency Participation contract with FDOT.

The Proposer’s Checklist can be obtained at the Emergency Management Office, 20859 Central Avenue East, Room G40 (located in the basement of the Courthouse), Blountstown, Florida. All questions concerning the checklist can be addressed to Angie Smith (850) 674-8075.

Plans and specifications may be viewed or obtained at Preble-Rish, Inc., 20684 Central Ave. East Blountstown, FL 32424, (850) 674-3300. Cost for plans and specifications will be $50 per set and is non-refundable. Checks should be made payable to Preble-Rish, Inc. Questions concerning the plans or specifications should be addressed to Kristin Brown P.E. (850) 674-3300.

This project is federally funded with assistance from the Florida Department of Transportation (FDOT) and the Federal Highway Administration (FHWA). Conflicts of interest will be governed by FDOT Procedure No. 375-030-006 (Restriction on Consultants Eligibility to Compete for Department Contracts).

Letters of interest will be received until 3:00 p.m. Central Standard Time, on Monday, November 5, 2012, at the, Calhoun County Courthouse, Clerk of Court Office, 20859 Central Avenue East, Room 130, Blountstown, Florida.

Please indicate on the envelope that this is a sealed bid, for the “Altha Sidewalk CEI Services”.

NOTICE TO RECEIVE SEALED BIDS

The Calhoun County Board of County Commissioners will receive sealed bids from any qualified person, company or corporation interested in providing construction services for the following project:

ALTHA SIDEWALK PROJECT

This project will include the construction of 4,265± LF of 6’ wide sidewalk on North Street, Fuqua Circle and Smith Street within the Altha community. Detectable warnings, thermoplastic striping, and minor drainage improvements are also included.

Plans and specifications can be obtained at Preble-Rish, Inc., 20684 Central Avenue East, Blountstown, Florida 32424, (850) 674-3300 (For questions, please call Kristin Brown at (850) 674-3300). The bid must conform to Section 287.133(3) Florida Statutes, on public entity crimes.

Completion date for this project will be 120 days from the date of the Notice to Proceed presented to the successful bidder.

Liquidated damages for failure to complete the project on the specified date will be set at $200.00 per day.

Please indicate on the envelope that this is a sealed bid, for the “Altha Sidewalk Project”.

Bids will be received until 12:00 p.m. Central Time, on Tuesday, November 13, 2012, at the Calhoun County Courthouse, Clerk’s Office, 20859 Central Avenue East, Room 130, Blountstown, FL 32424, and will be opened and read aloud on Tuesday, November 13, 2012 at 2:00 p.m. CT. Cost for Plans and Specifications will be $50.00 per set and is non-refundable. Checks should be made payable to PREBLE-RISH, INC.
