GROUP HEALTH
CITY OF BLOUNTSTOWN

The City of Blountstown is requesting proposals for group health insurance for employees and eligible dependents. Please include in health proposal the plan’s accessibility in Calhoun County. A description of current benefits, rates and census information for quotation purposes may be picked up at City Hall. Assume an effective date of October 1, 2012.

Sealed proposals, subject to the minimum general conditions herein, will be received by the City of Blountstown, Florida. Eight (8) copies of sealed proposals marked “GROUP HEALTH,” must be received by 2:00 P.M., C.S.T., on Tuesday, August 14, 2012 at City Hall, Attention: City Manager, 20591 Central Ave. W., Blountstown, Florida 32424.

Proposals will then be opened August 14, 2012 and read aloud by the City Council during the Regular Session of the City Council on August 14, 2012 at 17262 NW Angle Street, Blountstown, Florida 32424. Bid opening will begin at 6:15 P.M., C.S.T.

The City Council reserves the right to accept the bid deemed in the best interest of the City and to reject any or all bids.

AN ORDINANCE ESTABLISHING AN OCCUPATIONAL LICENSE; APPLICATION PROCESS; PRESCRIBING LICENSE ISSUANCE PROCEDURES; FEES; LICENSE TERM; REQUIRED BUSINESS LOCATIONS; POSTING OF LICENSE; EXEMPTIONS; SETTING PENALTIES; CONFLICT; SEVERABILITY; ESTABLISHING AN EFFECTIVE DATE.

SHALL BE PRESENTED to the City Council, City of Blountstown, for second and final adoption at the Regular City council Meeting to be held Tuesday, the 14th day of August, 2012 at 6:00 p.m. in the City Council Meeting Room at 17262 NW Angle Street, Blountstown, Fl. 32424.

REQUEST FOR QUALIFICATIONS
RFQ Emergency Management Contractual Services

The Calhoun County Board of County Commissioners hereby requests sealed proposals from qualified individuals or firms to provide the following:

EMERGENCY MANAGEMENT
CONTRACTUAL SERVICES
CALHOUN COUNTY, FLORIDA

Interested parties should submit an original and 6 copies of proposals, sealed and clearly labeled “Sealed RFQ Emergency Management Contractual Services”. Proposals must be received by 12:00 P.M. (CT) on Monday, August 06, 2012 at the Clerk of Court Office at 20859 Central Avenue East, Room 130, Blountstown, Florida 324240, 2012. Proposals will be publicly opened on Monday, August 06, 2012 at 2:00 P.M. (CT) in the Emergency Operations Center (EOC) located in the basement of the Courthouse. The Selection Committee will then evaluate the proposals and present recommendations to the Calhoun County Board of County Commissioners. The meeting of the selection committee shall be open to the public.

Any proposals received after 12:00 P.M. (CT) on Monday, August 06, 2012, will be deemed ineligible and returned. The Calhoun County Board of Commissioners reserves the right to reject any and all documents received, waive formalities and re-advertise and award the RFQ in the best interest of Calhoun County.
The Board of County Commissioners does not discriminate because of race, creed, color, national origin or handicap status.

FALL 2010 CDBG IMPROVEMENTS PROJECT #013.068

NOTICE TO RECEIVE SEALED BIDS

The City of Blountstown will receive sealed bids from any qualified person, company or corporation interested in providing the requested services for the following project:

FALL 2010 CDBG IMPROVEMENTS

The project consist of the installation of approximately 2,800 LF of 6” forcemain, 3,400 LF of 6” watermain, and the replacement of 1 lift station. This project is being funded by a Small Cities Neighborhood Revitalization Community Development Block Grant.

Plans and specifications can be obtained at Preble-Rish, Inc., 20684 Central Avenue East, Blountstown, FL 32424, (850)674-3300. Cost for plans and specifications will be $ 100.00 per set and is non-refundable. Checks should be made payable to PREBLE-RISH, INC.

The substantial completion date for this project will be 150 days from the date of the Notice to Proceed presented to the successful bidder and fully complete with 180 days from the date of the Notice to Proceed.

Liquidated damages for failure to complete the project on the specified date will be set at $500.00 per day.

Please indicate on the envelope that this is a sealed bid, for the “FALL 2010 CDBG IMPROVEMENTS”

Bids will be received until 4:00 p.m. Central Standard Time, on August 28, 2012, at the City of Blountstown City Hall, 20591 Central Avenue West, Blountstown, Florida 32424, and will be opened and read aloud at 6:00 p.m. on August 28, 2012 at the scheduled City Council meeting. The City of Blountstown is an Equal Opportunity Employer/Handicapped Accessible/Fair Housing Jurisdiction.

The bid must conform to Section 287.133(3) Florida Statutes, on public entity crimes and all special and supplementary conditions of the CDBG contract award (see specifications). A Bid Bond in the amount of 5% of the Bid shall accompany the Bid. The City of Blountstown reserves the right to reject any and all bids. All Bids shall remain firm for a period of sixty days after the opening.

A mandatory pre-bid conference shall be held at the office of Blountstown City Hall, 20591 Central Avenue West, Blountstown, FL 32424 at 1:00 p.m. Central Standard Time, August 14, 2012. Prequalification packages must be received by Preble-Rish prior to the pre-bid conference.

All bidders shall comply with all applicable State and local laws concerning licensing registration and regulation of contractors doing business to the State of Florida.
