REQUEST FOR PROPOSALS
GROUP LIFE
CITY OF BLOUNTSTOWN

The City of Blountstown is requesting proposals for group life insurance for employees. A description of current benefits, rates, and census information for quotation purposes may be picked up at City Hall. Assume an effective date of October 1, 2009.

Sealed proposals, subject to the minimum general conditions herein, will be received by the City of Blountstown, Florida. Eight (8) copies of sealed proposals marked “GROUP LIFE,” must be received by 4:00 P.M. C.S.T., on Tuesday, July 14, 2009 at City Hall, Attention: City Manager, 20591 Central Ave. W., Blountstown, Florida 32424.

Proposals will then be opened July 14, 2009 and read aloud by the City Council during the Regular Session of the City Council on July 14, 2009 at 17262 NW Angle Street, Blountstown, Florida 32424. Bid opening will begin at 6:30 P.M., C.S.T.

The City Council reserves the right to accept the bid deemed in the best interest of the City and to reject any or all bids

INVITATION TO BID


Sealed bids will be received by the Calhoun County School Board until 5:00 P.M., CT, Tuesday, July 14, 2009, for the purchase of the following:

Bread Products for use in the school cafeterias for the bid period of August 1, 2009 through June 30, 2010.

Any additional information or bid forms required may be obtained at the School Lunch Office, 16651 SE River Street, Blountstown, Florida. Each bidder submitting a bid must use bid forms, otherwise bids will not be considered.

Any bid submitted after the specified hour and date will not be opened or considered. The Board accepts no responsibility for mailed proposals that arrive at the School Lunch Office after the above stated time, even though the envelope may reflect a postmark prior to the above date.

The Board reserves the right to accept or reject any or all bids. The decision of the Board will be final. No bid may be withdrawn by any bidder for a period of thirty (30) days after bids are opened.

BID NOTICE
STATE HOUSING INITIATIVES
“SHIP PROGRAM”

The Calhoun Board of County Commissioners will accept sealed bids on behalf of its State Housing Initiatives Partnership “SHIP” Program for the rehab of houses until 12:00 p.m. local time on Tuesday, July 21, 2009. Bids may be delivered to the Clerk’s Office in the Calhoun County Courthouse, Blountstown, Florida 32424. Bids must arrive before deadline. Bids will be opened on July 21, 2009 at 5:00 p.m. or as soon afterwards as possible. Bids must be sealed in a envelope marked “sealed bid for “ship rehab” and identified by the name of the firm, and the date and time of the bid opening. Bid sheets and specifications may be obtained by contacting Dan Clemons, 20859 East Central Avenue, Room 117, Blountstown, Florida, telephone (850) 674-2571.

A mandatory pre-bid conference will be held on Wednesday, July 8, 2009, at 9:00 a.m. local time in room 117 in the Calhoun County Building Department located on the 1st floor of the Calhoun County Courthouse. It is the responsibility of the contractor to purchase all permits deemed necessary. Bidders must provide proof of general liability and workman’s compensation insurance in the bid package. The insurance must be in force at the time of the bid opening. As an alternative to providing workman’s compensation insurance, the prospective bidder shall provide proof of worker’s compensation exemption. Any person claiming to be exempt shall be subject to an on-the-job inspection for proof of exemption and license of all workers on job site.

Minority contractors are encouraged to participate. Calhoun County is an equal opportunity jurisdiction. The Calhoun County Board of County Commissioners reserves the right to reject any and all bids and to waive any informality. The signing of the contract is contingent upon the approval of and/or release of funds by the Florida housing finance agency.
