Candace Samantha Cook, age, 32, of Blountstown, FL, passed away Friday, June 1, 2018 in Dothan, AL.

Candace was born on February, 24, 1986 in Blountstown to Ray Cook and Bonnie (Roy) Cook and had lived in Blountstown for most of her life. She worked as a CNA at Jackson County Hospital and Calhoun Liberty Hospital for several years. She was of the Protestant Faith. Candace was preceded in death by her maternal grandparents, Rudy and Dot Smola; paternal grandparents, Buck and Jeanette Cook; uncle, Joe Roy.
Survivors include her mother, Bonnie Cook of Blountstown, FL; father, Ray Cook of Altha, FL; children, Kaygan Cooper and his father, Jerome Cooper, Laila and Mackenna Ross and their father, MacKinzie Ross; paternal grandparents, Ernest and Faye McCardle of Altha, FL.; loving boyfriend, Leroy James of Cottondale, FL; siblings, Andy Cook and wife, Felicia of Houma, LA, Shelby Cook of Blountstown, FL, Sabry Cook of Altha, Fl. Veronica Taylor of Blountstown, FL; aunts and uncles, Roy Fay and wife, Melissa Fay of Fyffe, AL, Christopher Roy and wife, Rebekah of Clarksville, FL, Tonya Clark of Marianna, Fl, Joyce Hughes of Massachusetts; cousins, Lindy and Alisha Fay of Fyffe, AL, Jody Roy of Marianna, FL, Morgan Searfoss and husband, Shane of Clarksville, FL and D. J. Green of Marianna, FL.; best friends, Shanna Cloud of Panama City, FL and Jessica Gates of Blountstown, FL; numerous other family and friends
Funeral services were held Tuesday, June 5, 2018 at 10:00 am (CDT) at Peavy Funeral Home Chapel with Reverend Larry Rogers officiating. Interment followed in New Shiloh Cemetery in Altha.
All arrangements were under the direction of Marlon Peavy at Peavy Funeral Home in Blountstown, FL. 850-674-2266.
