STATE HOUSING INITAITIVES
“SHIP PROGRAM”

The Calhoun Board of County Commissioners will be accepting sealed bids for rehabilitation and septic tanks on behalf of its State Housing Initiatives Partnership Program “SHIP” until 12:00 p.m. local time on Tuesday, August 19, 2014. Bids may be delivered to the Clerk’s Office in the Calhoun County Courthouse, Blountstown, Florida 32424. Bids must arrive before deadline. Bids will be opened at 6:00 p.m. or as soon afterwards as possible. Bids must be sealed in an envelope marked “sealed bid for ship rehab” or “septic tanks” and identified by the name of the firm, and the date and time of the bid opening.

A mandatory pre-bid conference will be held on Thursday, August 14, 2014, at 9:00 a.m. local time in room 313 in the Calhoun County Building Department located on the 3rd floor of the Calhoun County Courthouse. Bid sheets and specifications were available at the meeting. It is the responsibility of the contractor to purchase all permits deemed necessary. Bidders must provide proof of general liability and workman’s compensation insurance in the bid package. The insurance must be in force at the time of the bid opening. As an alternative to providing workman’s compensation insurance, the prospective bidder shall provide proof of worker’s compensation exemption. Any person claiming to be exempt shall be subject to an on-the-job inspection for proof of exemption and license of all workers on job site. Copies of all contracting licenses must also be included in the bid package.

Minority contractors are encouraged to participate. Calhoun County is an equal opportunity jurisdiction. The Calhoun County Board of County Commissioners reserves the right to reject any and all bids and to waive any informality. The signing of the contract is contingent upon the approval of and/or release of funds by the Florida housing finance agency.

Calhoun County has received $350,000 from the State of Florida for the Rehabilitation of Homes and Emergency Repair.

Rehabilitation - This program provides repair assistance to eligible applicants for their homes. Eligible applicants who qualify for assistance and receive up to $10,000 in repair assistance (emergency repair and home repair combined) will be in the form of a grant, with no recapture provision. Eligible applicants who qualify for assistance and receive over $10,000 in repair assistance (emergency repair and home repair combined) will be in the form of a five-year zero-percent deferred payment loan. The five-year deferred loan will depreciate at a rate of 20% per year. Upon sale or transfer of property within the five-year loan term will result in a pro-rated repayment. Eligible homeowners will be selected on a first-come, first served basis. Preference will be given to elderly individuals, over the age of 62 years of age, and/or persons with special needs as defined in SHIP Rule 67.37.002 (13). Property must be owner-occupied. A verification of ownership, income and payment of property taxes will be conducted to determine the eligibility of applicants. SHIP funds may not be used for rehabilitation of mobile homes

Maximum award for Emergency Repairs shall not exceed $10,000.00

I. INTRODUCTION
Calhoun County School District is requesting written proposals from qualified construction firms to provide professional At-Risk Construction Management Services for the Budgeting and Construction Phase on the New Altha School.

II. It is the County’s intention to employ the Construction Firm at Risk to provide overall Project Construction Management, including Pre-Construction Services, Cost Benefit Studies, Information Management, Construction for Scope of Work and overall Project Management during the Construction on a cost plus a fee basis, with a guaranteed maximum price.

III. PROPOSAL INSTRUCTIONS AND GENERAL INFORMATION
Proposal Submissions: Submit seven (7) copies of a written proposal no later than 1:00 PM, CST on Wednesday, September 17.

Proposals must be responsive to the requirements and questions of the Request for Proposal.

Reservations: Calhoun County School District reserves the right, during the whole process, to reject any and all proposals, to negotiate changes in the new scope of work or services to be provided, and to otherwise waive any technicalities or informalities.

Method of Selection: Proposals shall be reviewed by the construction management review committee, which has been appointed by the Calhoun County School Board. The construction management review committee shall review the qualifications (which shall include, but not be limited to, the eight items listed below) of interested firms. Subsequently, the construction management review committee shall develop a short list of firms. The short listed firms will be invited to make presentations to the construction management review committee. Following the presentations, the construction management review committee shall make a recommendation to the school board. Upon acceptance of the recommendation from the construction management review committee, the Calhoun County School Board may enter into negotiations or entertain bids with the selected construction management firm.

Please respond by including but not limiting your response to the following:

1. Company name and length of time in business.
2. Company location.
3. Availability of time to start and complete project within Owner’s requirements.
4. Insurance carrier and applicable coverage.
5. Qualifications of staff to be utilized on this project with names, short resumes, length of time with firm and previous clients served.
6. Names of five (5) previous School Clients with phone numbers and contact person.
7. Description of previous experience, to include budget, final cost, time schedule, change orders, etc.
8. Experience working on an occupied school campus.
