NOTICE TO RECEIVE SEALED BIDS
The City of Blountstown will receive sealed bids from any qualified person, company or corporation interested in constructing the following project:
SAM ATKINS PARK
GROUND STORAGE TANK
The project will include the construction of a new 125,000 gallon potable water ground storage tank, associated site work, and all necessary yard piping. Tank materials are restricted to concrete or stainless steel.
Plans and specifications can be obtained at Preble-Rish, Inc., 20684 Central Avenue East, Blountstown, Florida 32424, (850) 674-3300. The bid must conform to Section 287.133(3) Florida Statutes, on public entity crimes.
Completion date for this project will be 180 days from the date of the Notice to Proceed presented to the successful bidder.
Liquidated damages for failure to complete the project on the specified date will be set at $100.00 per day.
Please indicate on the envelope that this is a sealed bid, the bid number and what the bid is for.
Bids will be received until 2:00 p.m.CST, on Tuesday, February 11, 2014 at Blountstown City Hall, 20591 Central Avenue West, Blountstown, Florida 32424, and will be opened and read publicly on Tuesday, February 11, 2012 at 6:00 p.m. CST. All Bids shall be submitted in an envelope clearly marked “Sealed Bid – SAM ATKINS PARK GROUND STORAGE TANK”. The City reserves the right to reject any and all bids.
Cost for Plans and Specifications will be $50 per set and is non-refundable. Checks should be made payable to PREBLE-RISH, INC.

“SHIP PROGRAM”
The Calhoun Board of County Commissioners will accept sealed bids on behalf of its State Housing Initiatives Partnership “SHIP” Program for the rehabilitation on houses until 12:00 p.m. local time on Tuesday, February 4, 2014. Bids may be delivered to the Clerk’s Office in the Calhoun County Courthouse, Blountstown, Florida 32424. Bids must arrive before deadline. Bids will be opened at 6:00 p.m. or as soon afterwards as possible. Bids must be sealed in an envelope marked “sealed bid for ship rehab” and identified by the name of the firm, and the date and time of the bid opening.
A mandatory pre-bid conference will be held on Thursday, January 23, 2014, at 9:00 a.m. local time in room 313 in the Calhoun County Building Department located on the 3rd floor of the Calhoun County Courthouse. Bid sheets and specifications will be available at the meeting. It is the responsibility of the contractor to purchase all permits deemed necessary. Bidders must provide proof of general liability and workman’s compensation insurance in the bid package. The insurance must be in force at the time of the bid opening. As an alternative to providing workman’s compensation insurance, the prospective bidder shall provide proof of worker’s compensation exemption. Any person claiming to be exempt shall be subject to an on-the-job inspection for proof of exemption and license of all workers on job site.
Minority contractors are encouraged to participate. Calhoun County is an equal opportunity jurisdiction. The Calhoun County Board of County Commissioners reserves the right to reject any and all bids and to waive any informality. The signing of the contract is contingent upon the approval of and/or release of funds by the Florida housing finance agency.

The Calhoun County Board of County Commissioners hereby requests sealed proposals from qualified individuals or firms to provide the following:

Interested parties should submit original and 6 copies of proposals, sealed and clearly labeled “Sealed RFQ Emergency Management Contractual Services.” Proposals must be received by 2:00 P.M. (CT) on Friday, January 31, 2014 at the Clerk of Court Office at 20859 Central Avenue East, Room 130, Blountstown, Florida 32424. Proposals will be publicly opened and evaluated on Tuesday, February 4, 2014 at 5:00 P.M. (CT) in the Calhoun County Commissioners Meeting Room.
Any proposals received after 2:00 P.M. (CT) on Friday, January 31, 2014, will be deemed ineligible and returned. The Calhoun County Board of Commissioners reserves the right to reject any and all documents received, waive formalities and re-advertise and award the RFQ in the best interest of Calhoun County.
The Board of County Commissioners does not discriminate because of race, creed, color, national origin or handicap status.
Anyone wishing to obtain RFQ documents or have any questions may contact the Emergency Management Office at 850-674-8075. Any questions concerning the RFQ specifications should be directed to Mrs. Angie Smith, Emergency Management Director.

All proposals must be submitted in a sealed package with the following written on the outside of the package: “RFQ – Emergency Management Contractual Services”, due date, and RFQ title clearly marked on the outside. If more than one package is submitted they should be marked 1 of 2, etc.
The proposal shall be signed by a representative who is authorized to contractually bind the Contractor.

B. Understanding of the County’s needs. Provide a narrative related to your understanding of the County’s needs, requirements and objectives as it relates to this proposed contract and your overall approach to those needs. This shall include previous work completed within the County and familiarity with the departments and personnel.
C. Qualifications/Experience & Company Profile - Provide a detailed technical narrative demonstrating your firm’s knowledge and experience of the procedures inherent in the scope of work as indicated.
D. References – A list of current and former major accounts along with contact person’s name and phone number(s). This list should include accounts that represent company’s experience with counties/entities of similar size and exposures as Calhoun County.
