MEDICAL RECORDS

The administration of the estate of Marion C. Eldridge Sr., deceased, whose date of death was July 7, 2010; file Number 10-31-PR is pending in the Circuit Court for Calhoun County, Florida, Probate Division, the address of which is 20859 Central Avenue E, Blountstown, Florida 32424.
All patients of Dr. Marion Eldridge or M.C. Eldridge Chiropractic located at 17019 SE Main Street, Blountstown, Florida 32424 are hereby given notice that the medical records, chiropractic records, x-rays and other such documents now in the possession of the Personal Representative/Curator of this estate will be destroyed after November 16, 2012 pursuant to Chapter 460, Florida Statutes. Any patient wanting a copy of their medical records must send a request in writing to the address listed below before November 16, 2012.

The City of Blountstown is requesting sealed bids for the removal and disposal of an above ground gas/diesel fuel storage tank, to include pumps and canopy. Tank capacity is approximately 5,000 gallons gas, 8,000 gallons diesel. Tank and related components were installed in 2009. Tank can be seen at the City Complex located at 17262 NW Angle St, Blountstown, FL 32424. Sealed bids marked “FUEL TANK” must be received by 2:00 P.M. C.S.T., on Tuesday, October 9, 2012 at City Hall, Attention: City Manager, 20591 Central Ave. W., Blountstown, FL 32424. Bids will then be opened and read aloud during the Regular Session of the City Council on Tuesday, October 9, 2012 at 17276 NW Angle Street, Blountstown, Florida 32424.
