RFQ Emergency Management Contractual Services

The Calhoun County Board of County Commissioners hereby requests sealed proposals from qualified individuals or firms to provide the following:

EMERGENCY MANAGEMENT CONTRACTUAL SERVICES
CALHOUN COUNTY, FLORIDA

Interested parties should submit an original and 6 copies of proposals, sealed and clearly labeled “Sealed RFQ Emergency Management Contractual Services”. Proposals must be received by 12:00 P.M. (CT) on Monday, August 06, 2012 at the Clerk of Court Office at 20859 Central Avenue East, Room 130, Blountstown, Florida 324240, 2012. Proposals will be publicly opened on Monday, August 06, 2012 at 2:00 P.M. (CT) in the Emergency Operations Center (EOC) located in the basement of the Courthouse. The Selection Committee will then evaluate the proposals and present recommendations to the Calhoun County Board of County Commissioners. The meeting of the selection committee shall be open to the public.

Any proposals received after 12:00 P.M. (CT) on Monday, August 06, 2012, will be deemed ineligible and returned. The Calhoun County Board of Commissioners reserves the right to reject any and all documents received, waive formalities and re-advertise and award the RFQ in the best interest of Calhoun County.

FALL 2010 CDBG IMPROVEMENTS PROJECT #013.068

NOTICE TO RECEIVE SEALED BIDS

The City of Blountstown will receive sealed bids from any qualified person, company or corporation interested in providing the requested services for the following project:

FALL 2010 CDBG IMPROVEMENTS

The project consist of the installation of approximately 2,800 LF of 6” forcemain, 3,400 LF of 6” watermain, and the replacement of 1 lift station. This project is being funded by a Small Cities Neighborhood Revitalization Community Development Block Grant.

Plans and specifications can be obtained at Preble-Rish, Inc., 20684 Central Avenue East, Blountstown, FL 32424, (850)674-3300. Cost for plans and specifications will be $ 100.00 per set and is non-refundable. Checks should be made payable to PREBLE-RISH, INC.

The substantial completion date for this project will be 150 days from the date of the Notice to Proceed presented to the successful bidder and fully complete with 180 days from the date of the Notice to Proceed.

Liquidated damages for failure to complete the project on the specified date will be set at $500.00 per day.

Please indicate on the envelope that this is a sealed bid, for the “FALL 2010 CDBG IMPROVEMENTS”

Bids will be received until 4:00 p.m. Central Standard Time, on August 28, 2012, at the City of Blountstown City Hall, 20591 Central Avenue West, Blountstown, Florida 32424, and will be opened and read aloud at 6:00 p.m. on August 28, 2012 at the scheduled City Council meeting. “SHIP PROGRAM”

The Calhoun Board of County Commissioners will accept sealed bids on behalf of its State Housing Initiatives Partnership “SHIP” Program for the rehabilitation on houses until 12:00 p.m. local time on Tuesday, August 21, 2012. Bids may be delivered to the Clerk’s Office in the Calhoun County Courthouse, Blountstown, Florida 32424. Bids must arrive before deadline. Bids will be opened at 5:00 p.m. or as soon afterwards as possible. Bids must be sealed in an envelope marked “sealed bid for ship rehab” and identified by the name of the firm, and the date and time of the bid opening.

A mandatory pre-bid conference will be held on Thrusday, August 9, 2012, at 9:00 a.m. local time in room 117 in the Calhoun County Building Department located on the 1st floor of the Calhoun County Courthouse. Bid sheets and specifications will be available at the meeting. It is the responsibility of the contractor to purchase all permits deemed necessary. Bidders must provide proof of general liability and workman’s compensation insurance in the bid package. The insurance must be in force at the time of the bid opening. As an alternative to providing workman’s compensation insurance, the prospective bidder shall provide proof of worker’s compensation exemption. Any person claiming to be exempt shall be subject to an on-the-job inspection for proof of exemption and license of all workers on job site.

1st Public Hearing Notice

The Town of Altha is considering applying to the Florida Department of Community Affairs (DCA) for a FFY 2012 Small Cities Community Development Block Grant (CDBG) of up to $600,000. These funds must be used for one of the following purposes:
1. To benefit low and moderate income persons;

2. To aid in the prevention or elimination of slums or blight; or

3. To meet other community development needs of recent origin having a particular urgency because existing conditions pose a serious and immediate threat to the health or welfare of the community and where other financial resources are not available to meet such needs.

The categories of activities for which these funds may be used are in the areas of housing, neighborhood revitalization, commercial revitalization, or economic development and include such improvement activities as acquisition of real property, loans to private-for-profit business, purchase of machinery and equipment, construction of infrastructure, rehabilitation of houses and commercial buildings, and energy conservation. Additional information regarding the range of activities that may be undertaken will be provided at the public hearing. For each activity that is proposed, at least 70% of the funds must benefit low and moderate income persons.

In developing an application for submission to DCA, the Town of Altha must plan to minimize displacement of persons as a result of planned CDBG activities. In addition, the Town is required to develop a plan to assist displaced persons.

A public hearing to receive citizen views concerning the community's economic and community development needs will be held at Altha Town Hall, 25586 North Main St, Altha, FL 32421 on Tuesday, August 14, 2012 at 6:00 P.M. The public hearing is being conducted in a handicapped accessible location.
